Yes, I nearly forgot about that link, but I was able to get it corrected. If you like, you can test the form and make sure it works for you. I am not sure what browser you use. I use several to test in, but incase it is a code flaw between browsers it may help.

I just tested it out using Firefox and it seemed to work perfectly. Let me know if you didn't receive the test.
